Perioperative nursing management of an adult patient with giant right-sided Bochdalek hernia: a case report

A 19-year-old man suffered from exertional dyspnea caused by a huge right-sided Bochdale hernia, which contained part of the liver, gallbladder, and colon, and caused approximately 70% compression of the right lung. The multidisciplinary team developed individualized perioperative plans including rehabilitation based on self-efficacy theory with respiratory training and nutritional support. During surgery, a system of surgical risk early warning, rapid response, accurate pressure injury risk assessment, dynamic positioning, continuous temperature monitoring, and target temperature management to prevent hypothermia have been implemented. Postoperative care included securing the airway, sequential respiratory support, early monitoring of gastrointestinal functions, and progressive pulmonary rehabilitation. The patient was discharged after 32 days without major complications. Follow-up at 3, 6, and 12 months confirmed complete recovery without recurrence. Complex individualized perioperative nursing interventions optimized recovery during right-sided diaphragmatic hernia repair in an adult.
