The new innovative CMS model MAHA ELEVATE (Make America Healthy Again: Enhancing Lifestyle and Evaluating Value-based Approaches Through Evidence) supports evidence-based interventions focused on nutrition, physical activity, lifestyle and holistic care for Original Medicare patients[1][2][3]. The model will provide approximately $100 million for 3-year cooperative agreements for up to 30 proposals that include services not currently covered by Medicare[1][2][4][5]. Each proposal must include a nutrition or physical activity component and demonstrate documented evidence of the safety, efficacy, and cost-effectiveness of the intervention[1][3][4]. Three awards are reserved for interventions targeting dementia[3][5]. The first cohort starts on September 1, 2026, the second a year later, with CMS issuing a funding announcement in early 2026[1][2][3]. Participants must collect and report data on clinical outcomes, quality, and costs, and funding must not be for food or services already covered by Medicare[1][3][5]. The model complements conventional medicine and collects data for future changes in reimbursements[2][4][6].
